Domine Jesu Christe qui me creasti
redemisti et preordinasti ad hoc quod sum,
tu scis quid de me facere vis;
fac de me secundum voluntatem tuam cum misericordia.
Amen

English translation

Lord Jesus Christ who created me,
redeemed [me] and pre-ordained that which I am,
you know what you wish to make of me;
do with me according to thy will, with compassion.
